On the obverse of the commemorative medal is the Peter and Paul Fortress with the spire of the cathedral, depicted by the Neva waves. Before the Peter and Paul Fortress, in the foreground are anti-tank barriers, the so-called “hedgehogs.” In the upper part on both sides of the spire of the Peter and Paul Cathedral are the dates - "1944" and "2004". On the reverse side of the medal, there is an inscription - “In honor of the 60th anniversary of the complete liberation of Leningrad from the fascist blockade”. Awards of golden color with a diameter of 32 mm are made of a tombac.
